Joska Healthcare Adult Foster Care Program (GAFC)
The Joska Healthcare Group Adult Foster Care (GAFC) program is a MassHealth funded program that provides personal care services in either a community or assisted living setting to individuals who are elderly and/or disabled. GAFC provides assistance with Activities of Daily Living (ADLs) and instrumental Activities of Daily Living (IADLS) under the professional oversight and support of a Registered Nurse and a case manager
Joska GAFC Personal Care Attendants Services:
Assistance with Activities of Daily Living” (ADLS)
- Bathing
- Personal Hygiene
- Dressing
- Grooming
- Assistance with Mobility Transfer & Exercise
Joska GAFC Personal Care Attendants Services:
Assistance with Instrumental Activities of Daily Living (IADLS)
- Shopping and Errands
- Meal Preparation
- Laundry and Light Housekeeping
- Transportation arrangements
Joska GAFC Personal Care Attendants Services:
Assistance with Medication Reminders
- PCA's do not administer medication but can remind the client to take medication at the right time.

Eligibility Requirements for Joska Healthcare Group Adult Foster Care Program
- The GAFC Program is for MA state residents who are 22+ years old.
- They must have a physical, medical, mental, or cognitive issue that prevents them from living independently due to a need for assistance with daily living activities.
